(You must update the form every two years.) NOTE 1: Be sure to use ONE COMPANY NAME CONSISTENTLY when completing the form and all other future dealings with us (e.g. letterheads, letters of interest, invoicing, proposals, balance sheets, audit reports, etc.) unless you inform us of an official name change via revisions to the DOT-CS-100 Form and data requested by our Finance Office. (See instructions attachment.) NOTE 2: OUR FILE SPACE IS LIMITED here in the consultant contracts group. Please keep in mind that most likely your company brochures, pamphlets, booklets and notebooks will ultimately be sent to our office for storage. Multiples of the same item will probably be thrown away. The best suggestion might be to send one set of them to the TDOT Director or person over the particular discipline(s) to which they relate so they will have at least one opportunity to be reviewed by the most appropriate person before being stored in our files. NOTE 3: BE CONCISE. The DOT-CS-100 Form is the most basic document you submit. Please use it to communicate your expertise but with care not to pack it with unrequested material, full resumes and an overwhelming volume of job experience documentation. A thick form containing multitudes of project may not be nearly as effective as one listing a reasonable number that show PRECISELY AND CONCISELY what your firm can, has and is doing.
